*Dying Matters encourages you to take 5 simple steps to make end of life experience better for all*

Step one... Write your will

Step two… Record your funeral wishes so your family know what you would like

Step three… Plan your future care and support – for example, set up a Lasting Power of Attorney and write an advance care plan

Step four… consider registering as an organ donor – decide whether you want to join the organ donor register or even leave your body to science, and share that decision with your loved ones.

Step five… Tell your loved ones your wishes – you may find it easiest to start talking about what you wouldn’t want rather than what you do. Talk to your family and friends, as well as your GP or other health & care professionals.
